In conjunction with an Instructional Design project created for Technology Specialists in the Church of Jesus Christ of Latter-day Saints, I created the following eLearning module in Adobe Captivate. The training was a proof of concept for two modules that had been designed for them and included several free form responses and knowledge checks.
